Request an Intake Packet

When you request the intake packet, our office receives your request and need to be process it. The packet is not automatically sent to you immediately.

Since automated systems have a higher chance of infringing on your privacy and can’t react to your specific situation or needs properly, we process your request manually.

We respond to packet requests as soon as we can which is typically the next business day, but it can be a couple of days if we are out of town for a conference or receive a number of requests.
